The Dining Services Server is responsible for providing prompt and accurate service to residents and guests in the dining venues at National Lutheran Communities. Some responsibilities include: - Performs a variety of services in the dining areas and kitchen including waiting on assigned tables, taking food and drink orders and setting and busing tables. - Greets residents/guests cordially and takes their order within a time frame of 5 minutes after they have been seated - Reviews menus with customers and present any special items of the day - Takes accurate food and beverage orders at the table - Retrieves food orders from the kitchen and complete plate preparation as specified on plate garnishment/presentation guidelines - Serves complete meal to all residents/guests in the dining room in a timely fashion - Stocks service station prior to meal service completely and correctly - Performs side work correctly as assigned according to the side work procedure guidelines and to the level required on the sanitation checklist - Clears dirty dishes in a timely manner from all tables - Utilizes the point of sale (POS) system to place all food orders and track resident meals - Reports any accidents and/or incidents to the Coach on duty or their designee immediately - Assists Dining Service team members and supervisors as needed

The Speech Therapist (Speech Language Pathologist (SLP)) administers speech therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Clinical Services Director. Speech therapy services are furnished only by or under the supervision of a qualified speech pathologist or audiologist. Some responsibilities include: - Improves or maximizes the communication of the patient. - Returns the individual to optimum and productive living within the patient’s capabilities.   - Periodically participates with all other home care personnel in patient care planning. - Provides full range Speech Language Pathology Services as ordered by physician. - Directs and supervises personnel, as assigned. - Takes initial history and makes initial evaluation. - Performs all skilled procedures. - Consults with physicians regarding change of treatment.   - Writes reports to physicians regarding patient’s progress. - Instructs patients and family members in home programs. - May be required to present an in-service to the myPotential at Home team members. - An initial evaluation, including plan of treatment and goals, must be completed and submitted to the physician for approval. A recertification by physician every 60 days is necessary if further treatment is to be continued. A progress note is written each visit. A reevaluation is written when expected duration of treatment is reached. The Registered Nurse (RN) has primary responsibility and accountability in assisting in the provision of quality comprehensive nursing care to the residents/guests. The core RN Staff Nurse functions including assessing, planning, implementing and evaluating the care of a designated group of residents. Some responsibilities include: - Responsible for managing all assigned personnel, supplies and equipment on the unit and for promoting team work. Ensures continuity and quality care on the assigned unit - Ensures appropriate in-service education programs are conducted for nursing personnel as determined by resident needs and as required by state/federal regulations - Provides strong supervisory and coaching skills for nursing assistant and medication aide staff - Partners in the position on-boarding of Nursing Service Staff; ensure appropriate training on NLCS standards of Nursing Services practices during the orientation of new employees - Assesses residents’ physical and psychosocial status and environmental needs of assigned residents. - Identifies needs for other services/disciplines for residents and intervenes appropriately to secure these services - Documents admissions and completes ongoing nursing assessments on assigned residents - Develops measurable goals, admission and staff plans with physicians, resident, families and interdisciplinary team members. Documents and communicates plans appropriately - From continual assessment, reviews and revises resident care plan needs, concerns and goal changes. - Communicates changes with RN Care Coach. Identifies necessary and appropriate nursing interventions pertinent to the plan of care - Demonstrates clinical competence to assess the physical, spiritual, psychological and emotional needs of each resident. Intervenes appropriately, requesting physician or other department intervention as appropriate. Ensures that documentation is completed as required - Collaborates and communicates, with tact and sensitivity, with other team members and departments to coordinate an interdisciplinary approach to resident care. Identifies and interprets the psychological reaction of the residents and takes appropriate action within the time frame necessary. Documents nursing interventions and resident response to care accurately, timely and completely - Administers resident treatments and procedures in accordance with physician orders and NLCS nursing policies and procedures - Administers medication and IV fluids skillfully, safely and as required - Handles crisis situations in a prompt, precise and professional manner. Actively participates in the direct delivery of resident care. Completes rounds to ensure that residents are receiving the appropriate nursing care in an appropriate time frame. Ensures that a safe, comfortable, clean environment is maintained - Organizes information and communicates with physicians and supervisor regarding relevant patient care issues. Coordinates activities on the assigned unit to ensure appropriate functioning of the unit and continuity of quality care. Transcribes/verifies physician orders, coordinates testing, treatments and procedures with other departments - Follows up on nursing care to ensure residents satisfaction. Ensures standards of patient care are utilized and delegates responsibilities as defined by policy or necessary to ensure appropriate resident care. - Monitors and assists assigned staff as necessary to ensure completion of all tasks properly and in a timely manner - Partners in answering residents call light and assists with resident other nursing staff as needed - Documents resident care and observations for assigned residents - Assists in direct resident care such as handling or moving residents - Responds to emergency and/or life threatening situations and provides assistance as required - Participates in implementation of admission and discharge of residents as delegated

The Occupational Therapist (OT) administers occupational therapy to clients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Clinical Services Director. Some additional responsibilities include: - Improves or minimizes residual physical disabilities of the client - Works to return the individual to optimum and productive living within the client’s capabilities - Periodically participates with interdisciplinary team in client care planning - Provides prescribed occupational therapy - Performs all skilled procedures as required - Consults with physicians regarding change in treatments - Writes reports to physicians regarding client’s progress - Instructs clients and/or family members in home programs and fine motor movement exercises - Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with myPotential defined timeframes as assigned - Presents an in-service education to myPotential staff as requested - Assists physicians in evaluating level of function - Helps develop the plan of care and revises as necessary  - Prepares clinical and progress notes - Advises and consults with clients, family members and other myPotentialteammembers

The Physical Therapy Assistant (PTA) administers physical therapy to clients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Physical Therapist (PT). Some responsibilities include: - Understands and adheres to established NLCS policies and procedures. - Provides physician prescribed physical therapy under a plan of care established by the PT. - Improves or minimizes residual physical disabilities of the client. - Returns the client to optimum and productive level within the client’s capabilities. - Participates with all other health care personnel in client care planning. - Performs all skilled procedures as ordered by physician and according to the plan of care established by the PT. - Consults with PT regarding change in treatment. - Instructs clients and family members in home programs and activities of daily living. - Participates in in-service programs and presents in-service programs as assigned. - Participates in performance improvement activities as assigned. - Attends all client care conferences as scheduled. - Prepares medical records, progress notes and updates care plans for each client visit in a timely manner as per Agency policy. - Performs services planned, delegated and supervised by the PT. - Assists in preparing clinical and progress notes. - Participates in educating the client and family.
